Score each form across seven dimensions

• Business criticality: what breaks if the form is unavailable?
• Usage: how often is the form submitted, edited, or referenced?
• Complexity: rules, repeating sections, code, data connections, attachments, and conditional logic.
• Workflow dependency: approvals, notifications, item updates, escalations, or external system updates.
• Data model: SharePoint list, library, XML, SQL, web service, Dataverse, or other source.
• Owner readiness: whether business and technical owners can validate the replacement.
• Target fit: Power Apps, Microsoft Forms, SharePoint form, Dataverse, archive, retire, or custom pattern.

Use the score to route the form
